According to work requirements, our hospital plans to select and purchase the sanitary material warehouse shelf project. Qualified units are welcome to apply and participate. The relevant matters are hereby announced as follows: 1. Procurement Project Content: Project Service Requirements: 77 sets of shelves in the sanitary material warehouse, with a budget of 89000 yuan. Please refer to Attachment 1 for detailed requirements. 2、 Registration requirements and relevant materials required for registration: (1) If the product manufacturers or authorized agents who participated in the bidding of Hongdu Traditional Chinese Medicine Hospital in Nanchang City in the past 3 years have dishonest behavior or bad records, or have not strictly fulfilled the contract, the hospital will not accept registration; (2) Suppliers with after-sales service issues will not be accepted for registration by hospitals; (3) The original and photocopy of the legal representative's identity certificate or legal representative's authorization letter, ID card stamped with the company seal; (4) The supplier shall provide a screenshot and print of the credit China query results, stamped with the official seal of the unit; (5) Suppliers are required to provide screenshots and prints of the query results for the list of serious illegal and dishonest behaviors in government procurement on the China Government Procurement Network, stamped with the official seal of the unit; (6) Copy of the Business License for Enterprise Legal Person, stamped with the official seal of the company; (7) Supporting materials for similar performance of the company and a list of users, stamped with the official seal of the unit; (8) Suppliers are required to provide proof of enterprise scale (small and medium-sized enterprise declaration letter), stamped with the official seal of the unit; (9) Capable of independently assuming civil liability and stamped with the official seal of the unit; (10) Having a good business reputation and sound financial accounting system, stamped with the official seal of the unit; (11) Having the necessary equipment and professional technical capabilities to fulfill the contract, stamped with the official seal of the unit; (12) Having a good record of paying taxes and social security funds in accordance with the law, stamped with the official seal of the unit; (13) Certificate of no major violations of laws and regulations or safety accidents in the past three years, stamped with the official seal of the unit; (14) Declaration letter meeting other conditions stipulated by laws and administrative regulations, stamped with the official seal of the unit; (15) After the registration is completed, a comprehensive review of the qualifications of the applying units will be conducted. Only after passing the qualification review can internal selection be carried out. According to the Notice of Jiangxi Provincial Department of Finance on Promoting the Supplier Qualification Credit Commitment System for Government Procurement (Gan Cai Gou [2023] No. 8), it has been decided to implement the supplier credit commitment system in the field of government procurement in our institute. Suppliers participating in our institute's government procurement projects can choose to use the commitment system as a substitute for the 9-14 qualification materials mentioned above. Please refer to Attachment 2 for the template details. )3、 The relevant materials required for the selection process within the institution: (1) All the materials required for the second item of registration (for the convenience of review, please attach the agency fee on the first page of the bidding document, and other registration materials should be attached in order after the agency fee table) need to be stamped with the official seal of the unit; (2) Provide service solutions, work ideas, etc. based on service needs; (3) The list of service users shall be stamped with the official seal of the unit; (4) The after-sales contact information and after-sales service commitment for this service; 4、 Registration Notice: 1.
