Our hospital plans to inquire and purchase medical ultrasound coupling patches within the hospital. We now invite interested qualified suppliers to participate and submit response documents before 2:30 pm on April 18, 2025 (Beijing time). 1、 Procurement content: Medical ultrasound coupling patch 2. Budget: 55000 yuan 3. Quantity: 5000 pieces 4. Evaluation method: Lowest price method (randomly selected if the lowest price is the same) 5. Procurement requirements (see Annex 3) 6. Quotation table: The quotation is one-time and does not accept second quotations (see Annex 2) 7. Qualification requirements: (1) Comply with Article 22 of the Government Procurement Law of the People's Republic of China. (2) This project does not accept consortia. (3) Supplier qualification requirements: If the supplier is a manufacturing enterprise, it must have a valid medical device production license (applicable to Class II and Class III medical devices); If the supplier is a business enterprise, it must have a valid medical device operation record certificate (applicable to Class II medical devices) or medical device operation license (applicable to Class III medical devices). 8、 Registration method and document acquisition method: Interested suppliers are requested to send an email to register. The registration period is from April 15, 2025 to 17:00 on April 17, 2025. The format table and content are not allowed to be adjusted or modified. Please directly fill in the project name and company name (* * * project * * * company registration) for the "subject" of the email, otherwise it will be considered invalid registration. 9、 Preparation and submission of response documents: (1) The response documents shall be bound into a booklet, in triplicate (one original and two copies), and the content of the documents must include a quotation table, valid qualifications and relevant supporting materials, after-sales service commitments, and each page of the response documents shall be stamped with an official seal and indicate consecutive page numbers; The outer packaging is fully sealed, indicating the contact person and contact information, and stamped with the official seal at the seal. (2) The response documents, whether written, printed, copied, or scanned, should be clear, tidy, and standardized. Otherwise, any adverse effects caused shall be borne by the supplier. (3) The response document must be submitted on-site at the opening time, and other methods such as mailing are not accepted. (4) Please send the original electronic scanned copy of the response document to the registration email within 1 working day after the response document is opened. (5) Provide samples (samples are an important evaluation factor, failure to provide them is considered invalid response) (VI) After the evaluation is completed, samples can be retrieved (within a period of three working days).
